Before the visit
Ask the person you support what they want help with. Write down their questions. Bring a current medicine list if the care team asks for it.
During the visit
Take notes with permission. Ask the care team to explain testing, costs, and next steps in plain language. The patient’s wishes should guide the discussion.
After the visit
Help keep appointments and contact details in one place. Ask the care team whom to call with questions. Share records only through a channel the care team approves.
